EMERGENCY INFORMATION

We are none of us getting any younger and as time goes by the likelihood of falling ill, or having an accident, when out-and-about can only get greater. But what would then happen? How would potential helpers know how to treat you? Without certain vital information treatment could be delayed or withheld with serious, if not fatal, consequences.

With these concerns in mind, we have designed a form for you to complete giving all the vital information required and we suggest that you fill this in and place it in an envelope marked ‘To be opened in a medical emergency only’ which you should then carry with you at all times. You can now download this form from the link at the top right hand corner of this page.

Of course, we all hope this is never required, but at least now you will be sure that, if the need arises, the right sort of help will be available as quickly as possible.
